CAHPS® Clinician & Group Survey

The CAHPS Clinician & Group Survey (CG-CAHPS) assesses patients' experiences with health care providers and staff in doctor’s offices. Survey results can be used to:

  • Improve care provided by individual providers, sites of care, medical groups, or provider networks.
  • Equip consumers with information they can use to choose physicians and other health care providers, physician practices, or medical groups.

To ensure that survey results are comparable across users, every CAHPS Clinician & Group Survey starts with a set of “core” items that everyone uses. Users can then customize their survey by adding supplemental items. Browse supplemental items for the Clinician & Group Survey.

Versions of the Clinician & Group Survey

Multiple versions of the CAHPS Clinician & Group Survey are available to meet users' needs. The CAHPS Database collects and reports both the 3.0 and the 2.0 versions of the Clinician & Group Survey. Review the instruments by selecting the PDF links.

Clinician & Group Surveys 3.0


Asks adults and children about their experiences with care from a doctor’s office in the last 6 months.

Clinician & Group 12-Month Survey 2.0


Asks adults and children about their experiences with care from a doctor’s office in the last 12 months.

Clinician & Group Visit Survey 2.0


Asks patients to report on their experiences with their provider at a recent visit. Only questions about getting access to care refer to the last 12 months.

Clinician & Group 2.0 Survey With Patient-Centered Medical Home (PCMH) Item Set


Asks patients to report on their experiences with primary care providers over the last 12 months. This expanded survey addresses a broader set of topics by incorporating the Patient-Centered Medical Home Item Set into the 12-Month Survey.
